Part Time Clinical Social Worker

Job Overview:

The Clinical Social Worker’s primary role is to deliver individual and group clinical services to older adults ages 60 and over living in the AmPark NORC community. The Clinical Social Worker is instrumental in furthering the agency’s mission to enhance the quality of life and maintain independence for older adults as they age in place in their community. The Clinical Social Worker works under the supervision of the Program Director.

Key Responsibilities, in coordination and consultation with the Director, include:

• Engages older adults in their homes, in the office, and in other community settings to gather information about their strengths, needs, abilities, and preferences
• Assesses older adults’ presenting problems using multidimensional psychosocial assessments
• Partners with older adults to establish a plan of care to address benefits and entitlements, advocacy, referral to community resources, problem-solving, and to provide supportive contacts
• Maintains regular contact with older adults to ensure timely follow-up to presenting problems
• Updates older adults’ contact information, consents, and assessment data on an annual basis
• Evaluates older adults’ plan of care to ensure that the services provided are effective
• Participates as an integral member of the NORC team, including case conferences and team meetings
• Provides regular updates to the Program Director about older adults’ presenting
  problems
• Responds to crisis situations as appropriate
• Assists with group programming
• Documents all services provided to and on behalf of seniors in the electronic database via detailed case notes and worker logs
• Maintains confidentiality of all client information and adheres to New York City Department for the Aging policies and professional standards of conduct
• Attends meetings and trainings with outside agencies as needed
• Other duties as assigned

Qualities and Qualifications: 

• LMSW required, LCSW preferred
• Bilingual English/Spanish or English/Russian preferred

Salary: $30,000 – $35,000 (commensurate with experience)

This is a part-time, in-person role, 18 hours per week located at the AmPark NORC. The AmPark NORC is a site of the Riverdale YM-YWHA.
